Students entering Preschool or Kindergarten must provide proof of a physical within a year of the first day of school.

Sports Physicals

A Sports Physical and a signed Concussion Form are required yearly for any student participating in a school sponsored organized sport (7th-12th grade). The physical and concussion forms must be on file before the student begins practice. To check the dates of your student’s last sports physical and concussion form, please contact:
